January 1, 2008 – December 31, 2008 Criteria
The alternative reporting criteria for quality measures data for measures groups reported for the January 1, 2008 – December 31, 2008, reporting period are 30 consecutive patients from whom the measures of one measures group apply. Consecutive patients must include at least two Medicare fee for service patients and may include some non-Medicare patients.
July 1, 2008 – December 31, 2008 Criteria
The alternative reporting criteria for quality measures data for measures groups reported for the July 1, 2008 – December 31, 2008, reporting period are 15 consecutive patients from whom the measures of one measures group apply. Consecutive patients must include at least two Medicare fee for service patients and may include some non-Medicare patients.

Request payment for 6 months (15 patients) OR 12 months (30 patients)
4. Add Patient Data:

5. Submit to CMS:
